General Information for New PatientsPhysio Performance is excited to help you along your health journey to meet your unique goals. Please reach out to us via email or set up a free consult call to ensure Physio Performance is right for you. Once we schedule you, then look out in your email for Patient Intake Forms which will need to be filled out prior to your first appointment.

Do I have to be Injured for Physical TherapyNo, you may simply have a goal of moving better, improving strength or mobility. Physical therapist are movement specialists who can help you meet your goal regardless of pain or injury. We can also assess your function with the goal of reducing your risk for an injury.

What if you don't take my insurance? (out-of-network questions)Many out-of-network insurance plans provide covered/reimbursed benefits. You will pay our cash rate of $150 for your evaluation and then $100/ treatment session. We supply you with a receipt of payment and a form with the information your health insurance needs to process your claim for reimbursement. It is your responsibility to know your out-of-network PT coverage. Please call your health insurance company and ask for your "out-of-network coverage". This is typically a percentage that you receive back. For example, if you have a 30% co-insurance for out-of-network services then you could be reimbursed 70% of your payment each visit covered from your insurance company, typically as a check mailed back to you.

What do I wear?Please wear clothes you would exercise in, ideally loose fitting clothing making it easier to examine the body part being treated.

What is your cancellation policy?We ask you give us 24 hour notice to cancel your appointment or you will be charged $50. We understand life happens, therefore, we allow a one-time forgiveness. After that we apply the $50 charge. We have to maintain this policy due to waitlist and competitive times to schedule appointments.

Do I need a referral for PT?No! Maryland allows for direct access with a doctor of physical therapy. You can come via "direct access". Your physical therapist may reach out to your physician/surgeon to best treat you and also to update them on your progress.

Will you come to my home?Yes, we supply in-home PT. Based on distance we may add a fee for travel.

How do I pay?We take all credit cards, HSA/FSA cards, and checks.
